Eligibility

Inclusion criteria

Inclusion criteria: EF should be less than 40% Newly diagnosed heart failure patients of age >= 18 years The participants should have stable HFrEF and stable clinical status The participants should be stable on other drugs Participants should have been on aldosterone inhibitors beta-blockers and angiotensin converting enzyme inhibitors / angiotensin receptor blocker/ angiotensin-neprilysin inhibitors before being initiated on SGLT2 inhibitors

Exclusion criteria

Exclusion criteria: Any patient with prior history of urinary tract infection or kidney disease with effective glomerular filtration rate (ml/min) less than 50 or prior history of diabetic ketoacidosis will be excluded Pregnant women No myocardial infarction in the last 4 weeks No major non-cardiac surgeries coronary artery bypass graft surgery or percutaneous coronary intervention should have been done for the patient in the last 4 weeks

This is a proof of concept study. The following study has not been done previously. mRNA profiling of the difference in expression levels before and three months after SGLT2i treatment will help us in gaining knowledge on the target genes. Understanding the genetic and molecular mechanisms of SGLT2i drug will help in paving path for better treatment strategies and also in drug development strategies. Timepoint: 0 Day to Day 90
