Inclusion criteria

Inclusion criteria: -Patient/Attendant Giving Consent -All Adult Patient Admitted >72 Hours In ICU , Either Directly Discharged to home or transferred to ward.

Exclusion criteria

Exclusion criteria: - Post Operative patient With surgery - Patient Discharged from ward within 24 hours of transfer
