Eligibility

Inclusion criteria

Inclusion criteria: All elective patients (Between 18 to 70 years of age), classified as ASA I or II, are scheduled for neurosurgery in the sitting position, and receiving ventilation with low tidal volume.

Exclusion criteria

Exclusion criteria: Patients with ASA >III, Any recurrent cardiac arrhythmia; reduced ventricular systolic function (ejection fraction <40%), BMI of more than 30 kg/m2, Chronic lung disease, Chronic kidney disease, and Pre-operative use of beta-blocking agents will be excluded.

To evaluate the reliability of short-term low PEEP challenge (SLPC)-induced decrease in stroke volume index (â?? SVI- SLPC) to predict fluid responsiveness in patients undergoing neurosurgery with lung-protective ventilation in the sitting position.Timepoint: Starting Point: At the point when the sitting position of the patient is achieved with hemodynamic stability. End Point: When hemodynamic parameters are noted after the fluid challenge.

â?¢ To compare the predictability of this method (â??SVI-SLPC) with that of traditional dynamic indices (SVV & PPV) to predict fluid responsiveness. â?¢ To determine appropriate best cut-off values over the receiver operator characteristics (ROC) curve. â?¢ To assess the correlation between (â??SVI-SLPC) and changes in SV induced by volume expansion (â??SV-VE).Timepoint: Starting Point: At the point when the sitting position of the patient is achieved with hemodynamic stability. End Point: When hemodynamic parameters are noted after the fluid challenge.
