Inclusion criteria

Inclusion criteria: Adult patients mechanically ventilated patients in NICU (either an endotracheal tube or tracheostomy tube) who have failed the initial weaning trial and meeting the inclusion criteria during the study period. Mechanical ventilation settings (Requiring fraction of inspired oxygen (FIO2) = 50% and Positive end expiratory pressure Diagnosis: severe traumatic brain injury, aneurysmal sub arachnoid haemorrhage, Guillain-Barre syndrome, Myasthenia gravis.

Exclusion criteria

Exclusion criteria: Presence of pneumothorax, chest tubes Body temperature more than 380 C Audible air leak around endotracheal tube/tracheostomy tube Glasgow Coma Scale ( GCS Serum albumin

Primary

MeasureTime frame
- To estimate the Resting energy expenditure (REE) and oxygen consumption (V ?O2) in NICU patients with difficult weaning from mechanical ventilation.Timepoint: All the study parameters will be recorded daily during the weaning trials for up to 7 days or earlier if the patient is totally of ventilator for continuous 24 hours.

Secondary

MeasureTime frame
To assess the influence of oxygen consumption (V ?O2) on weaning ratio (WR) in patients admitted at neurocritical care units. To observe the relation between DO2/V ?O2 balance, work of breathing(WOB) and weaning outcomes. To compare the changes in REE and V ?O2 between the neurosurgical and neurological patients during difficult weaning from invasive mechanical ventilation. Timepoint: similar time points as the primary objective
