Eligibility
Inclusion criteria
Inclusion criteria: Patients on 5 or more prescribed drugs with at least one FRIDs Ambulatory Willing to participate & give consent MoCA (Montreal Cognitive Assessment) with a score above 26 and can walk 10 meters or with a walking aid(walking stick, canes, callipers)
Exclusion criteria
Exclusion criteria: Critically ill patients, Patients with known neurological illnesses, cognitive impairment/ Dementia, Parkinsonism, stroke Elderly on homecare or special care Older adults using walkers, wheelchair
