Inclusion criteria
Inclusion criteria: 1. Patients suffering from two or more of the chronic conditions 2. Patients who are under usual care for the suffered chronic conditions. 3. Ability to comply with the study protocol 4. Voluntary written Informed consent
Exclusion criteria
Exclusion criteria: 1. Patients suffering from only one of the clinical conditions. 2. Inability to comply with the study protocol. 3. Severe mental health problems: schizophrenia, bipolar affective disorder, or other psychotic illnesses. 4. Life-threatening illnesses, malignant tumours, HIV etc. 5. Patients, at the time of enrolment, should not be suffering from any acute clinical condition that has no association with their chronic ailment
