Inclusion criteria

Inclusion criteria: 1. Heart Failure with LVEF 2. Age > 18 years 3. Atleast two months follow up in the center(or) 4. Atleast second visit following acute heart failure hospitalization.

Exclusion criteria

Exclusion criteria: 1. Refusal to consent 2. Failure to adhere to therapy
