Inclusion criteria
Inclusion criteria: 1 Age should be between 18-60years at the time of admission in psychiatry ward 2.Consecutive admitted patients meeting the criteria for either diagnosis of schizophrenia (F20), persistent delusional disorder (F22.0), psychotic mania (F30.2 & F31.2) and psychotic depression (F32.3 & F33.3) as specified by the ICD-10 Classification of Mental and Behavioural Disorder, Diagnostic criteria for research (ICD-10 DCR)18 with predominance of monothematic (delusional beliefs that are all related to a single theme) non bizarre delusion with or without hallucination 3.Patients should be drug free for last weeks during acute phase presentation 4.Patients should be cooperative for the assessment and accompanied by at least one informant.
Exclusion criteria
Exclusion criteria: 1) Duration of current psychotic mania and psychotic depression is more than 6 months. 2) Patients having any hearing or vision problems. 3) Co-morbid dependence on any psychoactive substance (except tobacco) and other major psychiatric illness (e.g., schizoaffective disorder, dysthymia, catatonic features) 3) Head injury, epilepsy or any structural brain lesion or organic brain syndrome (based on clinical examination and investigation reports), dementia any endocrinological illness or taking the treatment for the same. 4) Those patients who are not coming for follow-up after 1 month +one week or were not compliant with medication (i.e. below the level of assured/confirmed) 5) When current relapse is a breakthrough (reappearance of symptoms despite the continuation of an adequate dose of drugs)
