Eligibility

Inclusion criteria

Inclusion criteria: Inclusion criteria for patients with stroke 1. Have age 18 years or above 2. Have ischemic stroke 3. Be discharged from hospital within 10 Days 4. Have severity of stroke score ( within 16 to 42) by using national institutes of health stroke scale Inclusion Criteria for Caregivers: Caregivers who will be 1. 18 years or above 2. The primary caregiver at home. 5. Be able to understand Hindi 6. Be ready to become part of the research

Exclusion criteria

Exclusion criteria: 1. Unconsciousness with GCS > 8 2. Having complications of diabetes mellitus such as neuropathy and diabetic foot 3. Having existing cerebro-vascular conditions with physical disability like limb function 4. Having mental illness (anxiety disorder, post traumatic stress condition, psychosis, schizoaffective & schizophrenia illness) 5. Already having cognitive dysfunction (depression and delirium) and physical dysfunction before having stroke
