| There will be improvement in the quality of transfer process from critical care post implementation of the transfer model. There will be a difference between the pre & post implementation score. Caregivers will be learning more details abouts the patient current condition and the better way to take a good care of them. Family satisfaction rate will be more about the transfer process. Overall a better transfer care will be provided to all the transfer patients from ICU to ward. Timepoint: at interval of 15 days post discharge | — |
