Eligibility
Inclusion criteria
Inclusion criteria: 1. Ischemic stroke or ICH 2. 18 years or older 3. Within 16 weeks of the index event 4. ‘Probable stroke’ (high clinical suspicion of stroke based on acute focal neurological deficits in fitting with a neurovascular territory lasting for more than 24 hours, but without imaging confirmation of stroke) OR ‘definite stroke’ (brain imaging confirmed stroke [CT or MRI]).
Exclusion criteria
Exclusion criteria: 1. Patients will be excluded if they are deemed by the investigators to be unlikely to adhere to study processes. 2. If they have a neurological condition (besides stroke) that is associated with cognitive or functional decline 3. An established diagnosis of pre-stroke dementia.
