Eligibility

Inclusion criteria

Inclusion criteria: Adults aged >18 years Patients with Type 2 diabetes Patients under treatment for T2DM. HbA1c at baseline > 9.5% Patients who are willing to give written informed consent Patient with adequate knowledge to use a smartphone without assistance.

Exclusion criteria

Exclusion criteria: Patient with Type 1 Diabetes Gestational diabetes Patients with Latent autoimmune diabetes in adults (LADA) Patients who refused to give written informed consent Patients who are unable to use smartphones without assistance.

MeasureTime frame
To evaluate the diabetes care programs impact on HbA1c, FBS, and PPBS reduction in people with diabetes after three months.Timepoint: 2 timepoints

MeasureTime frame
1. To assess the changes in body weight, blood pressure, lipids, medications, diabetes distress & quality of life. 2. To assess adherence and compliance to self-management among people with diabetes.Timepoint: 2 timepoints
