Eligibility

Inclusion criteria

Inclusion criteria: Critically ill children 2 years to mechanical ventilation( oxygenation index >= 4 or oxygen saturation index >= 5 with chest infiltrates

Exclusion criteria

Exclusion criteria: Neuromuscular disease Contraindications for trans-esophageal catheter insertion Active uncontrolled esophageal or gastric bleeding Traumatic brain injury Extubation within 24 hours of intubation
