Inclusion criteria
Inclusion criteria: To analyze the health information management practices followed by Diabetes patients in management of their diabetes before and after the interventional mechanism ï?? Patients with in the age group of 40 to 65 who is diagnosed with Type 2 DM for a period of >= 2 years who are using glucometer ï?? HbA1c values between 7 to 10 %. ï?? Active patient at KH endocrinology OPD. ï?? Literate ï?? Patients using android smart phones.
Exclusion criteria
Exclusion criteria: ï?? Gestational Diabetes. ï?? Patientâ??s with other terminal illness
