Eligibility
Inclusion criteria
Inclusion criteria: Residing in the study area for most of the last 6 months
Exclusion criteria
Exclusion criteria: Likely to move out of the study area in the next one year or Does not understand the nature of study due to mental or physical derangement such as deafness, blindness, inability to speak etc, Or does not understand either English or Hindi languages

| We would determine the proportion of participants diagnosed with diagnosed chronic illnesses who changed their doctor during the follow-up period. The chronic illnesses would include hypertension, diabetes, asthma, COPD, migraine etc.Timepoint: 12 months from enrollment for all participants.
