Inclusion criteria
Inclusion criteria: Cluster Level: 1.Villages under Rural field practice area of AIIMS Bhubaneswar. Individual level: 1. Persons with diagnosed hypertension and/or diabetes mellitus and /or dyslipidemia aged between 30-60 years. 2. Residing in the same hamlet of the village for last six months. 3. Patients willing to give written informed consent to participate in the study.
Exclusion criteria
Exclusion criteria: Cluster Level: 1.Villages under Rural field practice area of AIIMS Bhubaneswar with distance less than 5 kms from Community Health Centre. Individual Level Individual level: 1. Patients unable to communicate (b/o hearing loss, diagnosed psychosis etc.) 2. Patient with diagnosed cases such as myocardial infarction, stroke, heart failure, advanced liver disease. 3. Debilitated patients
