Eligibility

Inclusion criteria

Inclusion criteria: All Patients who are 18 years of age or older will be screened for AHRF, defined as tachypnea (respiratory rate > 25/ min), PaO2/FiO2 ratio Patients with AHRF who fail COT will be identified by any of the following criteria- 1. Worsening tachypnea (RR > 35) , 2. Oxygen saturation 3. Signs of respiratory distress( decreased consciousness, tachycardia, paradoxical respiratory movements, use of accessory muscles)

Exclusion criteria

Exclusion criteria: 1) Chronic respiratory failure, 2) Hemodynamic instability (systolic BP 0.1 mcg/kg/min or Dopamine requirement > 10mcg/kg/min ) 3) A do-not-intubate order 4) Anticipated life expectancy less than 90 days
