Inclusion criteria

Inclusion criteria: Age > 18 yearS Patients got hospitalised in Emergency medical unit of Department of Internal Medicine PGIMER, who need advance long term airways management will be assessed after three days of admission for need of tracheostomy as per treating physician decision and standerd protocol followed. Consent to participate in study.

Exclusion criteria

Exclusion criteria: Age Patients who are not willing to give consent for the study. Pregnant female COVID-19 positive patients Difficult neck anatomy â?? cervical spine injury, Neck mass (tumors), aberrant blood vessels Trauma Patients, cervical injury Surgical emergency Active skin infection of anterior neck

Primary

MeasureTime frame
I.Turnaround time â?? Time in a day from decision of tracheostomy to actual execution of tracheostomy procedure. II.Procedural time - Time in minute from incision to insertion Timepoint: At Base line, 24 hrs , 7th day and at 3 months

Secondary

MeasureTime frame
I.Bleeding during the procedure â?? Significant blood loss which required blood transfusion/lead to hemodynamic unstability II.Surgical site Infection - Surgical site visible pus / positive culture and sensitivity at 7th days post tracheostomy. III.Ventilator acquired pneumonia - Confirmed clinically / CXR IV. APACHE 2 & SOFA scores â?? It is scoring system to predict clinical outcome of critical patients. V.Tracheal Stenosis â?? Confirmed by Fiber optic bronchoscopy done after 90-days post tracheostomy VI.Mortality Timepoint: 3 month
