Eligibility

Inclusion criteria

Inclusion criteria: Age > 18 years. Intubated. Receiving Mechanical Ventilation in ICU > 48 hours. COPD Patients of type 2 respiratory failure. Patients with Maximum inspiratory pressure (PIMAX): 15 to 30 cm H2O. Rapid shallow breathing index (RSBI): 60â??105

Exclusion criteria

Exclusion criteria: Unable to perform test manoeuvre. Any pre-existing neuromuscular disease. Any Associated Pulmonary Disease. Acute coronary syndrome (ACS). Haemoptysis. Spinal cord injury above T8. Any skeletal pathology that impairs chest wall movements such as severe kyphoscoliosis congenital deformities or contractures. Poor general prognosis or anticipated fatal outcome.

Design outcomes

Primary

MeasureTime frame
weaning success. Duration of mechanical ventilation (days). Reintubation. Tracheostomy. Length of ICU stay and hospital stay (days). Mortality. Adverse events in patients. Timepoint: Till hospital discharge/death of the patient, whatever comes first

Secondary

MeasureTime frame
Arterial blood gas (ABG). Inspiratory muscle strength (Plmax). Rapid shallow breathing index (RSBI) i.e Lung compliance associated with adequate gas exchange and lower respiratory rate. Measurement of diaphragm thickness and function by ultrasound. Timepoint: Till hospital discharge/death of the patient, whatever comes first
