Eligibility

Inclusion criteria

Inclusion criteria: 1.Patients above age more than 18 years. 2.Patient intubated and mechanically ventilated for minimum of 24 hrs. 3.Patient planned for extubation as decided by treating consultant (resolving disease, adequate gas exchange, stable hemodynamics). 4.Patient on CPAP/PS mode and successfully passed Spontaneous breathing trail.

Exclusion criteria

Exclusion criteria: 1. Patient who is mechanically ventilated less than 24 hrs 2. Nonconsenting patients 3. Tracheostomised patient 4. Patient who fails spontaneous breathing trail 5. Poor sonographic window 6. Patient having diaphragm paralysis 7. Self Extubation

Design outcomes

Primary

MeasureTime frame
. To determine the predictive values of p0.1, parasternal intercostal muscle and diaphragm thickness in successful extubation from mechanical ventilation. . TO evaluate relation between the parasternal intercostal muscle thickness and the diaphragm thickness in mechanically ventilated patients planned for extubation. Timepoint: Half hour before extubation Extubation

Secondary

MeasureTime frame
Co relation of values of p0.1, parasternal muscle and diaphragm thickness with length of icu stay. Co relation of values of p0.1, parasternal muscle and diaphragm thickness with re-intubation outcome. Timepoint: total length of ICU stay
