Eligibility
Inclusion criteria
Inclusion criteria: Patients - Patients aged between 18 and 99 years with clinical diagnosis of hypertension and diabetes since at least 6 months, with or without complications; - who owns smart phone or any of their family member owns the smart phone; - who gives consent to participate in the study. Physicians - Physicians who have at least five years of post MD experience and have additional interest in developing technology for better patient care. Researchers - Researchers with at least five years of experience either with area of expertise in the health domain/software developer/methodology expert. We will invite software developer who has previously developed apps.
Exclusion criteria
Exclusion criteria: We will exclude pregnant women, Gestational diabetes, seriously ill and terminally ill patients
