Eligibility

Inclusion criteria

Inclusion criteria: Individuals aged 18 years and above having type 2 diabetes mellitus who are residing in study area.

Exclusion criteria

Exclusion criteria: Patients not willing to give consent or those who are critically ill will be excluded from the study. Patients who clearly state that they will shift residence within the next 1 year will be excluded.

Change in HbA1c levels pre and post intervention and monthly fasting blood sugar readings will be considered as the measure for effectiveness of colour coded diabetes monitoring chart.Timepoint: The HbA1c outcome will be measured at baseline and after 1 year. The monthly fasting blood sugar will be measured at baseline and once every month for a period of 1 year
