Eligibility
Inclusion criteria
Inclusion criteria: 1. hypercapnia ( pco2 > 45 on admission ) or hypoxia ( p/f 2. Requiring advanced respiratory support during icu admission ( respiratory support defined as NIV , or HFNO or invasive mechanical ventilation ) 3. Vasopressor requirement 4. HD for AKI 5. Delirium for > 48 hours
Exclusion criteria
Exclusion criteria: 1. Age 2. Pregnancy 3. MMSE 4. Patients offered palliative care 5. patients who had primary brain injury that was likely to result in conscious or cognitive disorder (e.g., traumatic brain injury, subarachnoid hemorrhage, acute stroke, post cardiac arrest, meningitis, and encephalitis); 6. patients with pre-admission diagnosis of dementia; 7. patients who received home ventilation prior to admission; 8. patients with end-stage cancer; 9. patients with withdraw/withhold status; 10. expected death within 24 h; 11. second or subsequent admission to ICU during the study period; 12. ICU stay
