Inclusion criteria
Inclusion criteria: INCLUSION CRITERIA Cluster Level: 1.Villages under Rural field practice area of AIIMS Bhuabaneswar 2. Persons with diagnosed hypertension aged between 30-60 years 3. Residing in the same hamlet of the village for last six months. 4. Patients willing to give written informed consent to participate in the study.
Exclusion criteria
Exclusion criteria: EXCLUSION CRITERIA Cluster Level: 1.Villages under Rural field practice area of AIIMS Bhuabaneswar with distance less than 5 kms from Community Health Centre. Individual Level 2. Patients unable to communicate (b/o hearing loss, diagnosed psychosis etc.) 3. Patient with diagnosed cases such as myocardial infarction, stroke, heart failure, advanced liver disease. 4. Debilitated patients
