Eligibility

Inclusion criteria

Inclusion criteria: Inpatients of age > 18 years admitted to the medicine department Patients with polypharmacy ( >5 prescription medications) Patients suffering from one or more of the following conditions - Diabetes, Hypertension, Ischaemic Heart Disease, Asthma & COPD and Rheumatoid Arthritis

Exclusion criteria

Exclusion criteria: cancer patients organ transplant recipients patients suffering from psychological problem(s) those who were unable to participate in the assessm
