Inclusion criteria

Inclusion criteria: Knee pain and crepitus Persistent pain minimum 3 months prior to recruitment Madhayam Kostha of patients Patients Age 40 to 70 years Moderate-to-severe pain on walking Patients who are fit for Snehana and Matra Basti and are willing to sign the informed consent will only be included in the study.

Exclusion criteria

Exclusion criteria: Patients who have systemic disorder which might interfere by panchakarma procedures. Patients who are contraindicated for Matra Basti BMI index more than 41 Dyslipidemia Known cases of major illnesses like Tuberculosis, AIDS, Anorectal malignancy Secondary osteo-arthritis due to rheumatoid arthritis, tuberculosis, haemophilia gout , trauma. Patient who have not giving informed consent Knee osteoarthritis due to trauma .

Primary outcome is reduction in pain, stiffness and difficulty in walking in cases of Janu Sandhigata Vata.Timepoint: 42 days
