Inclusion criteria

Inclusion criteria: Patients above 60 years of age, Both Gender hospitalized and also patients coming to the outpatient department during the study period and who are able to grand consent for participation in the study.

Exclusion criteria

Exclusion criteria: Patients below 60 years of age, who are diagnosed with any of critical or terminal illness ,Older adults who have any form of visual, hearing and cognitive impairment and who are unwilling to consent for their participation in the study.
