Inclusion criteria

Inclusion criteria: All neonates of 27 to 34 weeks gestation admitted to NICU with respiratory distress syndrome within first hour of life and whose parents have given consent for participation in this study.Respiratory distress syndrome will be diagnosed by the presence of respiratory distress (Silverman-Anderson score of 3 or more), need of respiratory support to maintain preductal saturation >90% and suggestive Chest X ray findings (low volume lung, reticulo-granular appearance, air bronchogram and/or bilateral white out lungs).

Exclusion criteria

Exclusion criteria: 1. Critical congenital heart disease 2. Suspected Chromosomal abnormalities 3. Major congenital anomalies 4. Suspected Congenital lung malformation (CLE, CPAM, CDH etc) 5. Pulmonary hypoplasia, Pneumothorax, Pleural effusion 6. Features suggestive of Congenital pneumonia (clinical and lab features of early onset sepsis with suggestive maternal history like positive high vaginal swab or histological examination of placenta) 7. Features of frank chorioamnionitis 8. Babies who were given surfactant before lung USG could be done.

To test the predictive accuracy of a Lung Ultrasound score for the need of surfactant therapy in Indian preterm neonates of 27 to 34 weeks gestational age with respiratory distress syndrome.Timepoint: Day 1 of life

Secondary

MeasureTime frame
1.To compare Lung ultrasound score in neonates receiving surfactant and not receiving surfactant. 2.To compare area wise (Anterior, Lateral and Posterior) LUS score between neonates receiving surfactant and not receiving surfactant. 3.To compare LUS scores in neonates requiring repeat dose of surfactant. 4.To find best cut off score of LUS to predict need for surfactant therapy. 5.To find the gestational age adjusted cut off scores (â?¤30 weeks and 30 weeks) for surfactant administration.Timepoint: Day 1 and 2 of life;â?¢ To compare Lung ultrasound score in neonates receiving surfactant and not receiving surfactant. â?¢ To compare area wise (Anterior, Lateral and Posterior) LUS score between neonates receiving surfactant and not receiving surfactant. â?¢ To compare LUS scores in neonates requiring repeat dose of surfactant. â?¢ To find best cut off score of LUS to predict need for surfactant therapy. â?¢ To find the gestational age adjusted cut off scores (â?¤30 weeks and 30 weeks) for surfactant administration.
