Eligibility

Inclusion criteria

Inclusion criteria: Individuals/Persons aged >=18 years, diagnosed for non-communicable diseases for at-least one year such as diabetes, hypertension and provided consent for the study will be included

Exclusion criteria

Exclusion criteria: Participants who cannot able to read Hindi/English. Patients with end-organ complication related to NCD Participants with intellectual disability Participants who didnâ??t give consent to the study

Primary

MeasureTime frame
1. Disease control status for diabetes, hypertension and hyperlipidemia 2. Awareness about NCD complications, frequency of screening and its danger signs Timepoint: Outcome will be assessed at baseline, 1 month, 3 month and 6 month post intervention

Secondary

MeasureTime frame
Life style modification for NCD such as healthy diet, physical activity, no-smoking and no-alcohol 2. Self-assessment of foot, oral cavity and breast 3. Regular follow-up and medication adherence to NCD Timepoint: Outcome will be assessed at baseline, 1 month, 3 month and 6 month post intervention
