Eligibility

Inclusion criteria

Inclusion criteria: Patients admitted in inpatient departments of General Medicine, Cardiology, Nephrology. Patients who were prescribed 5 or more drugs.

Exclusion criteria

Exclusion criteria: Death/discharge within 48 hours of hospitalisation. Discharge against medical advice. Patients admitted for pure infective etiology. Patients admitted in critical care unit. Incomplete patient case files. Age less than 60 years. Patients on less than 5 drugs.
