Inclusion criteria

Inclusion criteria: â?? Patients aged 60 years or more with multiple comorbidities â?? Patient with Smart phone using an iOS or Android operating system and able to handle it. â?? Giving Consent to participate in the study.

Exclusion criteria

Exclusion criteria: with Severe Disease condition and unable to undergo detailed evaluation

MeasureTime frame
Outcomes will be assessed on following parameters 1. Patient satisfaction will be measured on patient satisfaction scale (PSQ18), 2. Patient adherence will be measured on the MARS questionnaire. 3. Vitals monitoring (includes B.P/RR/HR) 4. Biochemical parameters to look for treatment responses (includes blood sugar /LFT/KFT/CBC, lipid profile, HBA1C) Timepoint: From recruitment after 12 weeks outcome will be assessed
