Intervention1: Early creation of pneumothorax: After achieving lung isolation, under aseptic precautions and ultrasound guidance, a fluid filled syringe will be attached to a 16 G canula and inserted

Eligibility

Inclusion criteria

Inclusion criteria: Adult patients undergoing minimally invasive thoracoscopic procedures requiring one lung ventilation where left sided Double lumen tube (DLT) is used for lung isolation

Exclusion criteria

Exclusion criteria: 1. Non consent 2. Coagulopathy 3. When lung adhesions are suspected: on CT scan, history of TB, previous surgery, pneumonia, chest wall involvement by tumor. 4. Patients with emphysematous bullae, preexisting pneumothorax or pleural effusion on lung imaging 5. FEV1 6. If spirometric lung function tests not done, patients with a diagnosis of significant Chronic Obstructive Pulmonary Disease (COPD) i.e. needing systemic steroids or theophylline will be excluded. 7. Cases where lung isolation not achieved i.e. lung slide present on non-ventilated side 8. Absence of lung slide at the site of needle insertion (signifying possibility of lung adhesions ) 9. Patients who are observed to have lung adhesions after insertion of thoracoscope.
