Eligibility

Inclusion criteria

Inclusion criteria: ASA III-IV adult patients (>= 18 year old) will be included in the study undergoing elective coronary artery bypass graft and single valve replacement surgery and simple congenital heart disease with median sternotomy as surgical approach

Exclusion criteria

Exclusion criteria: â?? Patients not giving consent â?? Concomitant cardiac procedures (e.g. aortic valve repair/replacement, mitral valve repair/replacement, aortic root replacements), double valve replacement. â?? Redo sternotomy. â?? Taking narcotic analgesics within 3 days pre-operatively or perioperative stress-dose steroids. â?? Chronic non-cardiac pain (e.g. lower back pain, fibromyalgia) requiring narcotic analgesics. â?? Allergy to amide-type anesthetics â?? Recurrent ventricular arrhythmias, low cardiac output requiring inotrope and/or intra-aortic balloon pump support, LVEF â?? Unable to understand how to use numeric pain rating scales. â?? Respiratory disease â?? High inotropics / vasopressor support intraop / after bypass.
