Inclusion criteria

Inclusion criteria: : ICU patients having high tidal volume of >8ml/kg during current pressure support ventilation and continuing to have high tidal volume >8ml/kg during pressure support of 6cm H2O

Exclusion criteria

Exclusion criteria: : VAS score >4 , RASS >+2, Temperature >102degree F, HCO3 50mmHg,P/F ratio lactation, contraindications to lumbar epidural catheter.

Design outcomes

Primary

MeasureTime frame
the serial effect of epidural morphine on p0.1 over timeTimepoint: every 6 hours for 48-72 hours ( end point determined by tolerance to Morphine)

Secondary

MeasureTime frame
To study the effect of epidural morphine on other parameters such as rapid shallow breathing index, diaphragm thickening index, minute ventilation, etCO2, arterial blood gases, vitals and sedation requirement. To study possible complications of the drug and / or procedure, recruitment rates and feasibility of study.Timepoint: Then serial measurement of p0.1 and other output variables are to be made at first 24 hrs at T0, T6, T12, T18, T24 intervals
