Eligibility

Inclusion criteria

Inclusion criteria: 1. Needs a complex TR coronary intervention requiring the use of a 7 Fr guiding catheter according to operator expertise. 2. Specific anatomic settings and techniques requiring large bore guiding catheter such as Left main (LM) disease, Complex bifurcation stenting, Chronic total occlusion (CTO), Rotational atherectomy, Lesion within vessels with severe tortuousity or large vessels ( >3.5 mm) 3. Patients undergoing emergent or elective PCIs 4. Hemodynamically stable 5. No history of prior ipsilateral brachial or transradial procedure

Exclusion criteria

Exclusion criteria: 1. Previous coronary artery bypass grafts (CABG) using both internal thoracic arteries, 2. Cardiogenic shock, 3. Unsuccessful radial puncture, 4. Patients having PCI with both radial and femoral access 5. Planned cardiac surgery within 1 month after PCI, 6. Arteriovenous shunt for renal dialysis on the procedure forearm 7. Raynaudâ??s phenomenon 8. Unable to follow up at 6 weeks
