Eligibility

Inclusion criteria

Inclusion criteria: • Moderate and late preterm babies (born between 32-37 weeks of gestation) • High-risk newborns those are in stable condition at the time of discharge • High-risk newborns admitted in NICU for issues related to prematurity

Exclusion criteria

Exclusion criteria: • Newborns with congenital anomalies • Newborns discharged against medical advice • Newborns discharged with unresolved medical issues

Primary

MeasureTime frame
•Reduction in readmission rate of high risk newborns •Improved growth and development of preterm babies in their environment Timepoint: 1. 7th day after discharge from NICU 2. 15th day after discharge from NICU 3. 30th day after discharge from NICU
