Inclusion criteria: Age >65 years. Gender: both (Male and female). Patient with chronic conditions. Patient willing to give consent to the study.

Exclusion criteria

Exclusion criteria: •Patients with COVID-19. •Patient with dementia or psychiatric disorder.
