Inclusion criteria

Inclusion criteria: 1.Severe AKI (KDIGO 3 classification) despite optimal resuscitation -Urine output of less than 0.3 mL/Kg/hour for 24 hours or more than 24 hours. -Less than threefold increase in serum creatinine level compared with baseline valve -Serum creatinine 4.0 mg/dl or more than 4.0 mg/dl. 2.Minimum 3 fold increase in Serum Creatinine level along with clinical decision by clinician. 3.Intention to provide full intensive care treatment for at least 3 day. 4.Written informed consent of patient or his legal representatives or the Authorized Representative. 5.Inclusion due to an emergency situation.

Exclusion criteria

Exclusion criteria: 1.Declined consent from guardian. 2.Patients with an increased bleeding risk (or) active bleeding due to vascular damage (Ulcer in G.I. tract, Intracranial Hemorrhage, Aneurysm of brain arteries). 3.Dialysis-dependent chronic kidney insufficiency. 4.Allergic reaction to one of the anticoagulants. 5.Need of therapeutic anticoagulation ( aPTT >60s, anti-Xa >0.6IE/mL, INR >2) 6.AKI caused by Glomerulonephritis, Interstitial Nephritis, Vasculitis or Urinary Tract Obstruction. 7.Haemolytic Uraemic Syndrome/ Thrombotic Thrombocytopenic Purpura. 8.Kidney transplant within the last 12 months. 9.Pregnancy and Nursing period.

Design outcomes

Primary

MeasureTime frame
To study the filter life span using Citrate and Bivalirudin as anticoagulation agent in CRRT.Timepoint: Every 1 hourly from initiation of CRRT to completion of CRRT.

Secondary

MeasureTime frame
1.To study the length of stay in ICU. 2.To study the bleeding and other complication of CRRT. 3.To study the transfusion requirement. Timepoint: 1- Every 24 hourly after get admitted in ICU till get discharged (For Length of stay in ICU) 2- After 1 hour of completion of CRRT. ( For Transfusion requirement, bleeding and other complications of CRRT).
