Eligibility

Inclusion criteria

Inclusion criteria: Acute ischemic stroke of anterior circulation Age >18yrs Eligible for Intravenous Thrombolysis

Exclusion criteria

Exclusion criteria: Any contraindication to thrombolysis: (Severe hypertension, Seizure at the onset, severe stroke, recent myocardial infarction, recent major surgery -History of Craniotomy, Burr-hole surgery, recent GI bleed) Posterior circulation stroke

Design outcomes

Primary

MeasureTime frame
To determine if addition of continuous 30 minutes of Transcranial Doppler ultrasound to IV thrombolysis with tenecteplase increases the proportion of individuals who have improvement in NIHSS of at least 4 points at 4 hours in acute ischemic stroke subjects of the anterior circulation in comparison to IV tenecteplase aloneTimepoint: To determine if addition of continuous 30 minutes of Transcranial Doppler ultrasound to IV thrombolysis with tenecteplase increases the proportion of individuals who have improvement in NIHSS of at least 4 points at 4 hours in acute ischemic stroke subjects of the anterior circulation in comparison to IV tenecteplase alone

Secondary

MeasureTime frame
Exploratory analysis of TCD waveforms in treatment group to identify spectral patterns and parameters corresponding to responders (those with improvement in NIHSS by 4 points) and compare with the non-responders Timepoint: Analysis of the TCD waveform pattern during the time of thrombolysis administration in acute ischemic stroke
