Inclusion criteria

Inclusion criteria: Inclusion: Patients of either sex, above the age of 40, diagnosed with chronic illnesses like hypertension, diabetes mellitus, cardiovascular disease etc, taking five or more drugs will be included in the study. Older patients will be defined as age above 60 at the time of data collection. For physicians: Inclusion: All doctors practicing in the department of medicine for at least 1 month will be included in the study, which includes senior faculties like professors, associate professors and junior faculties like assistant professors, senior residents and post graduates.

Exclusion criteria

Exclusion criteria: For patients Patients diagnosed with chronic disease but having less than five medications for treatment are excluded from the study Patients who have cognitive impairment, will be excluded from the study Patients not having access to internet are excluded from the study For Physicians Physicians not giving informed consent will be excluded from the study
