Eligibility

Inclusion criteria

Inclusion criteria: 1. Patients aged 18-65 years scheduled for decompressive craniotomy 2 Acute brain injury defined as intracranial injury presented within 48 hrs of insult (traumatic and non-traumatic) and diagnosed by imaging of the brain (CT/MRI) which require decompression surgery. 3 ASA 1-3

Exclusion criteria

Exclusion criteria: 1 Pre-existing anaemia with Hb concentration 2. Patients with coagulopathy with international normalised ratio (INR) >1.5. 3. Patients with history of severe cardiac, pulmonary, liver or renal disease, or obesity (body mass index >30) and pregnancy will be excluded from this study. • Patients who are hemodynamically unstable. • Head injuries associated with other organ/ tissue trauma • Patients / relatives refusing to give consent.

MeasureTime frame
1. Difference in the number of Red blood cell units transfused in acute head injury patients during decompressive craniotomy guided by SpHb (continuous non-invasive Hb monitoring) vs conventional estimated blood loss method.Timepoint: Total Red blood cells transfused through out the intraoperative period.

1. Incidence of post-operative infections 2. Length of stay in ICU 3. Difference in the mortality 4. Neurological outcome using E-GOS (Extended Glasgow outcome scale) Timepoint: 1. Incidence of post operative infections during first week of ICU stay 2.Total number of ICU days immediately in the immediate post operative period 3.Difference in the 30 post operative day mortality 4.E-GOS score at 90 days of post discharge from hospital
