Inclusion criteria

Inclusion criteria: Patients prescribed with anticoagulants in surgery, cardio-thoracic and cardiology units (Both in-patients and out-patients)

Exclusion criteria

Exclusion criteria: Critically ill patients Patients with psychiatric problems Pregnant women Patients not willing to participate in the study For PGx study: Other anticoagulants, inhibitors and inducers of warfarin excluded
