Eligibility

Inclusion criteria

Inclusion criteria: Patient of either gender aged 18 years and above undergoing emergency cranial surgery for acute TBI with moderate or severe GCS (12-3)

Exclusion criteria

Exclusion criteria: (i) No consent (ii) Pregnant patients (iii) Contraindication to study drugs (haemodynamically unstable patient, ASA 3 or 4) (iv) Patients with maxillofacial injuries (v) Patients with factors that will affect NIRS values (like underlying scalp haematoma or pneumocephalus) (vi) Airway contamination or likelihood of pulmonary aspiration (vii) Patients already intubated in casualty

Primary

MeasureTime frame
Effect of induction with Etomidate, Thiopentone and Propofol on rScO2Timepoint: Baseline at 0 hours Postoperative at end of 24 hours

Secondary

MeasureTime frame
(i) To evaluate the effect of anesthetic induced changes in systemic hemodynamic parameters,SpO2 and to correlate it with the changes in the bilateral rScO2 (ii) To correlate the volume of hemorrhage with rScO2 changes (iii) To see effect of anesthetic induction with etomidate on serum cortisol levels (iv) To see the prognosis of the patient at end of 30 days after surgery. Timepoint: 0 TO 30 DAYS
