Inclusion criteria

Inclusion criteria: Hospitalized patients above 18years of age admitted due to Chronic disease (such as Diabetes Hypertension Chronic kidney disease) and receiving at least two drug

Exclusion criteria

Exclusion criteria: pregnant women and lactating women patients who are not willing to give consent
