Eligibility

Inclusion criteria

Inclusion criteria: 1)Age more than or equal to 18 years 2) A citizen of India, who is a permanent resident of Kerala 3) heart failure patients as defined in ESC 2016.

Exclusion criteria

Exclusion criteria: 1. Refused to provide consent 2. patients enrolled in other cardiac rehabilitation programs 3. Bedridden patients
