Intervention1: Single arm Interventional study (Pre-post study design)in which Behavioral intervention will be given to all the enrolled patients.: The intervention components include health education
where adherence to prescribed treatment and their behavioral risk factors and biological risk factors
changes in CVD risk will be reassessed and individual case based counseling will be done at each follow- up visit. Control Intervention1: Not applicable: Not applicable

Eligibility

Inclusion criteria

Inclusion criteria: a)Patients with selected NCDs (cardiovascular diseases, diabetes mellitus & COPD) reporting to the outpatient department of selected health facilities b) Patients who are likely to stay in the study area for at least one year c) Those willing to take part in the study

Exclusion criteria

Exclusion criteria: Patients with other NCDs like carcinomas, psychiatric disorders, asthma

MeasureTime frame
Change of 10-year risk of developing cardiovascular disease (CVD) using the WHO/ International Society of Hypertension (ISH) risk-assessment charts for SEAR after one year of behavioural change intervention.Timepoint: At enrollment and at 3, 6 & 12 months of enrollment

Secondary

MeasureTime frame
Changes in behavioural characteristics (e.g. Tobacco smoking, excessive alcohol intake, physical inactivity and an unhealthy diet), biological characteristics (e.g. blood pressure control, blood sugar control, weight control) and adherence to prescribed treatment as compared to the baseline. Timepoint: At enrollment and at 3, 6 & 12 months of enrollment;The burden of patients with COPD reporting to the OPDs at the selected health centers and those diagnosed with COPD on opportunistic screening.Timepoint: At enrollment
