Eligibility

Inclusion criteria

Inclusion criteria: 1. Patients with CKD STAGE 4 OR 5, in predialysios or prevalent HD patients requiring new AVF 2. Feasible features for creation of RCF based on USG doppler ( Arterial diameter > 2mm, venous diameter > 2mm) 3. Ct venogram showing patent peripheral and mental veins in patients with history of previous central venous catheterisation or permanent catheter placement fistula

Exclusion criteria

Exclusion criteria: 1. Absence of adequate arteries and veins to allow autologous AVF on USG doppler 2. Patients with previous AVF failure 3. Patients with central venous stenosis on CT venogram 4. Inability to perform the physical exercise protocol due to physical or mental disabilities or lack of social support 5. Patients not willing to give informed consent

To compare primary failure rates in patients undergoing RCF surgery with PIE versus patients not doing PIETimepoint: 0, 2, 4, 6 weeks and 3 months
