Intervention1: Reconnection to ventilator: In one group patient will be extubated directly after passing SBT and Im another group pta will be reconnected to ventilator for 30 mons Control Intervention

Inclusion criteria

Inclusion criteria: Patient receiving mechanical ventilation for atleast 24 hr

Exclusion criteria

Exclusion criteria: Age less than 18 year Tracheomised patients Overwhelming secretion Out of protocol extubation Participants in other trails
