Eligibility

Inclusion criteria

Inclusion criteria: 1. Diagnosed case of diabetes within the age group 18 years & above. 2. Patients who are willing to participate. 3. Patient receiving insulin therapy for treatment of diabetes. 4. Diabetic patients with more than two records of HbA1c outside of recommended range >7% and HbA1c.

Exclusion criteria

Exclusion criteria: 1. Those who are not able to communicate regularly on phone. 2. Patient with gestational Diabetes. 3. Patient with serious or life threatening illness. 4. Patient involved in separate trial.
