Eligibility

Inclusion criteria

Inclusion criteria: 1. New York Heart Association(NYHA) classification â?? II to IV 2. Diagnosed with Heart Failure with reduced ejection fraction. 3. Left Ventricular ejection fraction of 4. Patients who are willing to consent for the study and able to read/understand the Kannada language. 5. Ability and willingness to operate the mobile phone for text message and respond for telephonic calling

Exclusion criteria

Exclusion criteria: 1. Patients with cognitive impairment 2. Patients who are critically ill unable to perform self-care activities. 3. Patients/ caregivers who do not use a mobile phone 4. Patients with other comorbidities having limited life expectancy such as chronic obstructive pulmonary, asthma, cancer, chronic kidney disease
